The wrist comprises of three different compartments which normally do not communicate with each other. Before injecting steroids, a clinical examination must be performed to assess in which of the compartments the synovitis is most pronounced. All compartments of the wrist are injected from a dorsal direction.
Steroid injections into finger-, thumb- or wrist joints might be indicated in rheumatoid arthritis (RA), osteoarthrosis (OA) or goat. It is important to exclude septic arthritis before injecting steroids! RA often produces fluid and swelling of the joints, which facilitates injection into the joint space.
The landmark for the interacarpal joints is the proximal pole of the Lunate bone, which is palpated with the wrist in flexion. The intercarpal joints are reached distally to the Lunate.
The landmark for the distal radio-ulnar joint is the ulnar head. The DRU-joint is reached radial to the ulnar head.
