The CMC-joint of the thumb

The saddle-shape of this joint might make it difficult to find the joint space. There are a lot of delicate structures around the joint, such as the radial artery, the radial nerve branches and the tendon of the Abductor Pollicis Longus. The joint space is most easily reached from a radial direction introducing the needle through the APL-tendon and deep into the CMC joint.

These joints have in principal the same design with collateral ligaments on both sides and a dorsal capsular pouch between the bone and the extensor tendons. Steroid injections are most easily performed into this pouch, provided that intratendineous injections are avoided.
